UNDERSTANDING OF OPC PARENT & PLAYER COMMITMENT AGREEMENT

By CHECKING THE BOX BELOW I understand that WSA Soccer belongs to Oklahoma Premier Clubs, and I am accepting an official position on an Oklahoma Premier Clubs team for West Side Alliance S.C. during this Seasonal Year (May 1 of this year, thru, June 1 of the subsequent year).

I understand the player is registered from the moment the player or the player’s agent executes the registration/enrollment process, and that at that time the player has made a one-year commitment to the OPC member club. 

By signing this form, I am acknowledging that in all cases and at all times, (on and off the field), I will conduct myself properly and always take personal responsibility for all my actions, especially as they affect my league, my club, my team, my fellow players, my coaches and my family.

Lastly, by accepting my position on this team, my parents and I are agreeing to the Player and Parent Expectations, WSA's Parent-Player-Club Covenant contained within this registration process, which includes codes of conduct and behavior standards for game-day, training, and time spent within and throughout the WSA space.  Ultimately I understand SPORTSMANSHIP is a minimum required standard and expected behavior at all times while participating with an OPC member club.
