Media Release & Permission (Youth Players)

Purpose
West Side Alliance Soccer Club (“the Club”) occasionally uses photos, videos, and related media to promote the Club’s programs and celebrate player participation. This form explains how your child’s name, image, and likeness may be used and details the permission granted.

What may be used

• Photographs and video recordings of your child taken during Club activities (games, practices, events)
• Your child’s name, team name, and age group (players under 11 may only use last name intials, ex: Alex J.) 

How it may be used

• Club social media accounts (e.g., Facebook, Instagram, X)
• Club website and digital communications (e.g., newsletters, emails)
• Printed materials (e.g., flyers, brochures, banners)
• Local media coverage or promotional materials

Important notes

• No personal contact information (address, phone, email) will be shared.
• Content will be used in a positive, respectful manner.
• The Club does not sell or transfer images for commercial use outside Club promotion.
• Once published online, the Club cannot fully control further sharing by others.
• Permission is voluntary and may be withdrawn at any time in writing.

Parent/Guardian Consent
I, the parent/guardian, grant permission to West Side Alliance Soccer Club to use my child’s name, image, and likeness as described above.

Effect for Full Seasonal Soccer Year, And Subequent Years
This permission remains in effect for the current seasonal year (May 1 of current year, through July 31 of subsequent year), and after the player graduates or moves on, unless withdrawn in writing.
